Use of neural networks for dosage individualisation of erythropoietin in patients with secondary anemia to chronic renal failure.

The external administration of recombinant human erythropoietin is the chosen treatment for those patients with secondary anemia due to chronic renal failure undergoing periodic hemodialysis. The goal is to carry out an individualised prediction of the erythropoietin dosage to be administered. It is justified because of the high cost of this medication, its secondary effects and the phenomenon of potential resistance which some individuals suffer. One hundred and ten patients were included in this study and several factors were collected in order to develop the neural models. Since the results obtained were excellent, an easy-to-use decision-aid computer application was implemented.

A case of naturally evolving gout in an elderly man

Gout is a common disorder in adults that can lead to severe organ decline, disability and impaired quality of life due to the formation of periarticular tophi. We report a case of massive tophaceous gout in a 78-year-old man with a 16-year-long history of untreated disease. The patient gradually became disabled, his renal function deteriorated, and he finally died of sepsis. Our case demonstrates that chronic gout not only affects the joints, but is also associated with organ function decline and can, even nowadays, lead to death. © 2011 Elsevier Ireland Ltd.

The aim of this study was to investigate the effect of hemodialysis duration on HDL(2) and HDL(3) compositions and lecithin:cholesterol acyltransferase (LCAT) activity in 58 patients on acetate hemodialysis using cuprophane membrane, after different periods of dialysis. Patients were divided into three groups-GI1 year, GII 1-5 years, GIII 5-13 years of dialysis and were compared with 22 controls. Increase by 34% of triacylglycerols (TG) was noted in GI and by 36% in GII versus GI. Hypertriglyceridemia was correlated with hemodialysis duration (HD) (r=0.75, P0.05). We evaluated, in a group of 41 CRF undialyzed subjects (29 men and 12 women, mean age 64.1 +/- 11.3 years), some parameters that reflect leukocyte activation (elastase, myeloperoxidase - MPO), plasma NO metabolites (NO(x)) and the oxidative status (lipid peroxidation expressed as thiobarbituric acid-reactive substances (TBARS) and total antioxidant status (TAS). INTRODUCTION: Nephron-sparing surgery (NSS) is mandatory for patients with renal tumors in both kidneys or in a solitary kidney in order to preserve renal function (imperative indication). NSS has also become the gold standard (elective indication) for small unilateral renal tumors (< 4 cm) with a normal contralateral kidney. We report the oncological long-term follow-up of NSS of our own series and discuss the results of the current literature.
